Key Services

Daily Personal and Self-Care Activities

Daily Personal and Self-care activities.

Our community supports team will support you in your own home or shared accommodation with your day to day self-care activities as directed by you to maximise your independence. 


Our key services include:


  • Support with self-care activities to include support with household tasks like cleaning, meal planning and preparation.
  • Assistance to manage life admin tasks such as planning for future appointments and tasks.
  • Assistance and prompting with personal care tasks to include showering and dressing. 
  • Transport to get where you need to go be it to a doctor's appointment, work or social outing. 

Community Access and Participation

 We provide assistance for you to get involved in activities of your choice in your local community. The supports can help you to take part individually or in a group depending on what your needs and goals are.   


Our community support services include: 


  • assistance to explore, connect and participate in recreational and leisure activities.
  • Support to maintain relationships and connections with family, friends and local community. 
  • Shopping assistance and bill payments. 
  • Attending GP and medical appointments.

Improved Living Arrangements

At Chona Care we understand that having access to appropriate and stable accommodation and community supports is key to maintaining your wellbeing. Working in partnership with you, our staff will support you with some of the following tasks:


  • Completing NSW housing application forms or for private rentals
  • Support to maintain rental ledger. 
  • Assistance to maintain tenancy obligations to include housing inspections and managing hoarding and squalor issues. 
  • Advocating and communicating with NSW housing and landlords on your behalf as needed.


Increased social and community Participation

We will engage in collaborative working with you, your family, allied health and clinical team to support with skill building and development in some of these areas:  


  • making friends and building safe  social connections in the community. 
  • Developing skills to promote future independence with tasks like shopping, attending appointments and paying bills. 
  • Researching, connecting and engaging in community supports and social activities.

Finding and keeping a job

Working in partnership with you our support staff will support you to:


  • Connect with employment agencies.
  • Assist to follow up on applications and job offers.
  • Assist to gather and submit required evidence to include, identification, qualifications and work screening. 
  • Support to attend work and with maintaining employment.  

Social skills development

Our staff will support you to develop social skills individually or in a group setting to enable you to participate and engage safely in social and communal activities.  Community supports offered include the following: 


  • Supported social outings to include eating out in a restaurant.
  • Support to attend and participate in social groups.
  • Staff assistance to practice and implement support plans and recommendations from your OT, Psychologist or clinical team.
